Most people, including other physicians have never met a psychiatrist. Adult
psychiatrists often do not really know what child psychiatrists do! To illustrate this
point, let's talk about a national scandal of sorts. In an ideal world the pediatrician sees
a lot of children that should be referred to a child psychiatrist; kids with attention
problems, behavioral problems, social anxieties, depression, hallucinations,
sudden changes in school performance, etc. But does the pediatrician refer them?
Hardly ever. They would rather do nothing. They may say, "Don't worry, he (or she)
will grow out of it." Or, they will retain a psychologist who generally knows very little
about medication, to recommend a psychotropic medication. Why does this situation
exist all over the United States?
The pediatrician will see 100 children with coughs. It behoves him to be conservative
in both diagnosis and treatment. He tells them to go home and rest; most of them will
get better. The pediatrician's job is to look for the needle in the haystack. The one or
two kids whose cough will proceed to pneumonia.
Now, when a child comes to see a psychiatrist, there's always something wrong
unfortunately. Child psychiatrists do not have to look for needles in a haystack because
they usually have a 3 alarm fire in front of them. A good child psychiatrist is
methodical and aggressive. But, the pediatrician sees the psychiatrist acting so
differently than him that he concludes, "These guys are out of control. They're not like
me. They are reckless about giving serious medications without regard to normal
development." But this theory does not make sense because that is what the
psychiatrist is always about: normal and abnormal development. So there we have it,
the pediatrician remains suspicious of the intentions and actions of the psychiatrist.
